Colchicine is an anti-inflammatory agent primarily used to prevent or manage symptoms of gout. Less commonly it may be used to treat pericarditis, familial Mediterranean fever, and other conditions.

Patients frequently buy colchicine as both a long-term, preventive treatment and as an as-needed treatment for gout attacks. While effective at reducing pain and inflammation during a gout flare-up, it is a poor substitute in treating pain from other sources, such as rheumatoid arthritis.

Managing Gout

There are three approaches to gout management with colchicine:

  • It may be taken in low doses long-term to prevent or reduce gout flare-ups;
  • It may be taken in larger doses as-needed to manage symptoms during a flare-up;
  • It may be taken in low doses long-term and taken in larger doses to manage attacks, but alternative flare-up treatments are preferred if possible in these cases.

Administration differs slightly depending on treatment approach:

  • As a preventive treatment, 0.6 mg (one tablet) is taken once or twice daily, depending on the frequency and severity of attacks.
  • When taken to abort an attack, 1.2 mg (two tablets) is taken, followed by additional 0.6 mg (1 tablet) doses at least 1 hour apart if needed; do not exceed 1.8 mg in one hour or 8 mg in 72 hours.
  • To abort an attack when also taking the medication long-term, consult a doctor for the ideal dosage; recommendations may vary considerably depending on individual circumstances.

As a preventive measure, colchicine reduces the frequency of attacks, the severity of symptoms during an attack, and the duration of attacks.

In non-gout conditions the medication is typically taken in small daily doses, however this can be highly variable depending on individual circumstances; patients are not advised to buy colchicine to treat other conditions without a diagnosis and treatment plan from a doctor.

Treating Attacks

When taking colchicine as an abortive treatment a total of 4 to 8 mg is typically needed to successfully stop the attack. Because the medication can be quite toxic if taken in large amounts, these doses must be achieved gradually, and limits may be determined by the patient:

  • After taking an initial large dose of 1.2 mg, wait at least one hour.
  • After 1 hour, take only a smaller dose of 0.6 mg, and wait another hour before repeating if needed.
  • Stop taking additional doses when stomach upset, vomiting or diarrhea occurs.
  • In subsequent attacks, take the amount that was previously administered before stomach upset or diarrhea occurred. For example, if vomiting occurred after taking 5 tablets, in the next attack stop at 4 tablets.
  • Always wait 1 hour between doses, and never exceed 8 mg.
  • If taking other medications to manage levels of uric acid, continue taking them normally.

Pain, inflammation and other symptoms of a gout flare-up should be greatly diminished within 12 hours, and the attack should stop completely within 24 hours.

Side effects are unlikely unless too much of the medication has been taken.
